Patient Fees/Insurance
Most workers’ compensation insurance is accepted
upon written agreement with the insurer.
Most evaluations for individual patients outside of
workers’ compensation are fee for service, due at
the time of the appointment, unless other
arrangements have been made. Payment by check,
credit card (Mastercard, Visa), or cash is
accepted. Patients will receive an itemized receipt
which they can submit to their insurance carrier(s)
for reimbursement.
Fees for services are based upon the nature and
complexity of the problem.
Communication
Dr. Craner will respond to initial inquiries via
email or telephone. E-mail communication with
established patients and clients is preferred and is
treated confidentially. For prospective or
non-established patients or clients, no medical
advice or consultation is offered over the internet
or telephone.
Privacy Notice and Confidentiality

patient information is treated as confidential. Confidential
or sensitive material may be transmitted
electronically via encrypted
or other form of secure communication.
Medical Records
Patients who wish to receive a copy of their medical
records may do so by completing the appropriate form
and sending it via email, fax, or US Mail to Dr.
Craner. There is no charge for electronic delivery
of medical records to patients. Attorneys, insurers,
and other parties who are requesting records must
produce a signed records release from each patient
and pay customary rates (currently $0.60) per page
and postage if records are not delivered
electronically.
